OSU Health Access Network (HAN) provides personalized care management for Oklahoma Medicaid members who need help coordinating their care. Experienced nurses and social workers partner with patients, families, and primary care providers to set health goals, manage chronic conditions, and connect individuals with community resources. HAN also offers health education, support groups, and assistance in overcoming barriers such as transportation, food insecurity, housing resources, and other needs. The program empowers patients with the knowledge and tools to stay healthy and actively manage their care.
