Occupational Health & Safety Program for Animal Contact


General Information

The Occupational Health and Safety Program at OSU-CHS is intended to fulfill and manage the institution’s responsibility to provide a safe workplace for faculty, staff, students, and visitors.

All individuals who have access to the vivarium are required to enroll in the program. You have been identified as someone who needs to enroll due to having access to live animals, carcasses, unfixed tissues, blood, body fluids, or waste.

Please follow the instructions and complete the enrollment forms. Please be advised that all health records are maintained confidentially. They are not shared beyond the occupational health office.

Who should enroll?

All faculty, staff, students, volunteers, IACUC members, and non-OSU collaborators who meet one of the following criteria are required to enroll in the OSU CHS Animal Care and Use Occupational Health Program:

  • Are listed as personnel on an Animal Care and Use Protocol.
  • Are involved in the care and/or use of live, vertebrate animals in research, testing, or teaching.
  • Work with animal carcasses, unfixed tissues, blood, body fluids, or waste.
  • Need access to the OSU CHS vivarium.
  • Serve as an IACUC member.

Instructions for Enrollment

Items needed for enrollment

Please fill out the Initial Enrollment form thoroughly.

1. Fill out the Animal, Biological, Chemical, and Radioactive Material Questionnaire

2. If you will be working with animals or animal products, then provide documentation of Tdap immunization. Tdap should be administered every 10 years.

3. If you will be working with human or non-human primate tissues, blood, or products, provide documentation of Hep B vaccine or titer.

You can fax them to our office at 918-561-1248. Please include a cover letter with your name.
