1. Letter of recommendation from the applicant’s  Program Director.
2. Two additional letters of recommendation from physicians in the United States including 1 from a Sports Medicine Physician.
3. Official medical school transcript
4. A complete CV
5. Personal statement
6. Official transcripts for all applicable examinations (COMLEX and/or USMLE).
7. Those qualified to train with us must successfully complete training within an ACGME or AOA accredited Family Medicine, Internal Medicine, Emergency Medicine, Pediatric, or PM&R residency program and secure full, unrestricted medical licensure in the state of Oklahoma prior to starting their Sports Medicine Fellowship.

Interviews

Eligible applicants will be considered for interview, ranking and appointment based on:

1. Academic performance
   
2. Grade point average
   
3. COMLEX-USA Level 1, 2CE, 2 PE, and 3 (For DO Applicants)
   
4. USMLE Step 1, 2CK and 2CS (For MD Applicants)
   
5. Ability
6. Overall preparedness
   
7. Communication skills
   
8. Personal qualities (such as motivation and integrity)
   
9. The interview, compatibility of the applicant with program philosophy and goals, academic record, letters of recommendation, and evidence of involvement in and commitment to sports medicine weigh heavily in the selection and ranking of applicants.
